Unauthorized access to or disclosure of Protected Health Information (PHI).
Concerns related to CMS documentation, billing inaccuracies, or financial misconduct.
Issues related to escalation protocols or patient safety.
Violations of the VitalGuard Code of Conduct or conflicts of interest.
Intentional bypassing of security controls, such as MFA or encryption.
VitalGuard strictly prohibits any form of retaliation against an individual who reports a suspected violation in good faith. Reporting a concern will not negatively impact your employment status, compensation, or relationship with VitalGuard.

If you choose to provide your name, VitalGuard will protect your identity to the maximum extent permitted by law.
Upon submission, you will be provided with a unique "Case Token." Please save this token; it allows you to communicate with our Compliance Officer and receive status updates without revealing your identity.
Pursuant to the Defend Trade Secrets Act (18 U.S.C. § 1833(b)), an individual shall not be held criminally or civilly liable under any federal or state trade secret law for the disclosure of a trade secret that is made in confidence to a government official or to an attorney solely for the purpose of reporting or investigating a suspected violation of law.
Individuals reporting a suspected violation of law may disclose trade secrets to their attorney and use that information in court proceedings, provided any document containing the trade secret is filed under seal.
